If you have a chronic condition, you may be eligible for a referral through a GP Chronic Condition Management Plan (GPCCMP). This entitles you to up to 5 Medicare-rebated sessions per calendar year ($63.40 Medicare rebate per session). Common eligible conditions include osteoarthritis (knee, hip, shoulder), chronic neck or lower back pain, rotator cuff injuries, post-surgical rehabilitation, and much more.
